Prescribe compounded medications with a pharmacist on the line

Send prescriptions the way you already do. We confirm every detail with your office before compounding, and a pharmacist is available for formulation questions.

Three ways to send a prescription

Choose whichever fits your workflow. Each one reaches a pharmacist.

  • E-prescribe

Search for PureHealth Compounding in your e-prescribing system. If we are not listed yet, send the referral form and we will provide the details.

  • Fax

Fax prescriptions to 267-212-1031. Include a callback number for the office.

  • Phone

Call 833-990-7873 and ask for a pharmacist. We can take prescriptions by phone where regulations allow.

Not sure where to start? The referral form takes two minutes and we call your office from there.

What happens after you send it

The same sequence for every prescription.

  1. Step 1: The physician submits a prescription

A licensed physician writes for the strength, dosage form and quantity an individual patient needs and sends it to the pharmacy. Patients can also ask their physician to refer them.

    E-prescribe, fax or phone

  2. Step 2: A pharmacist reviews and prepares the eligible formulation

The pharmacist confirms the details with the practice, checks that the preparation may be compounded for that patient, and prepares it under USP <797> for sterile preparations.

    One prescription, one patient

  3. Step 3: The patient receives guidance and the finished preparation

The preparation is labelled, released only after third-party testing passes and a pharmacist approves it, and sent with an informational brochure on how to use and store it. Refills follow the physician's instructions.

An informational brochure comes with every preparation

What a pharmacist confirms before compounding

We would rather ask a question than make an assumption. Expect a call if any of these are unclear.

  • Active ingredients and strengths
  • Dosage form and base, or that we may choose one
  • Quantity, directions and refills
  • Known sensitivities to inactive ingredients
  • Controlled substance and prescription monitoring requirements

Physician questions

For anything patient-specific, use the referral form and a pharmacist will contact your office.

Who can send prescriptions?

Any licensed physician, nurse practitioner or physician assistant, within their scope and state licensing. The physician is responsible for verifying that the patient has met all health requirements and is cleared to receive the compounded medication.

What should a compounded prescription include?

Patient name and date of birth, each active ingredient with its strength, the dosage form and base (or a note that the pharmacist may choose), quantity, directions, refills, and the reason a commercial product does not meet the patient's needs if your state requires it. If anything is missing we call the office rather than guess.

Can you tell me which bases and flavors you use?

Yes. A pharmacist can walk through the options for a given preparation, including considerations for sensitivities and stability. Note it on the prescription or in the referral form.

What is the turnaround?

It depends on the preparation and whether ingredients are in stock. We give you an expected date when we confirm the prescription, and we tell you promptly if anything changes.

Do you provide office-use or bulk preparations?

No. As a 503A pharmacy we compound for individual, identified patients under a prescription. Office-use stock comes from a 503B outsourcing facility, which is a different kind of operation.
