Who we are
Pure Health Compounding, LLC (“we”) operates this website from 343 Godshall Dr, Ste 500, Harleysville, PA 19438. This policy covers the website. Health information we hold as your pharmacy is also covered by our Notice of Privacy Practices.
What we collect
Referral form. Patient name, date of birth, email, phone, state, preferred contact methods, prescriber and practice details, the medication of interest and any message you write.
Prescription request form. Your name, email, phone, state, preferred contact methods, the formulations you are interested in, whether you have a prescriber and their name or practice if you give it, any message you write, and your typed name as a signature with the time you signed.
Clinic pricing form. Clinic name, practice type, states served, website, NPI, your name, role, email, phone, preferred contact methods, areas of interest, estimated volume and any message.
Technical data. Our server records a hashed form of your network address and your browser type when a form is submitted, to prevent abuse. The forms are protected by Cloudflare Turnstile, which may process limited device information under Cloudflare's privacy policy.
What we do not collect. This website does not use advertising cookies, tracking pixels or third-party analytics. We do not sell or rent personal information.
Why we use it
- To contact the patient and the prescriber about a referral and coordinate the prescription
- To have a pharmacist contact you about a formulation you asked about and coordinate with your prescriber
- To respond to clinic pricing requests
- To protect the site against spam and abuse
- To meet our legal obligations as a licensed pharmacy
How it is protected
Form submissions travel over an encrypted connection and are stored encrypted. Only pharmacy staff who need to work on your request can view it, each with their own account and two-step sign-in, and every view is logged. Internal notification emails never include the submitted details.
How long we keep it
Requests are kept while they are being worked on and for as long as pharmacy record-keeping rules require.
Your choices
