Prescriptions and compounded medications
Every compounded medication requires a valid prescription from a licensed prescriber for a specific patient. Listings on this website are informational; they are not offers to sell, and availability is confirmed by the pharmacy for each prescription. Compounded medications are not FDA-approved. They are prepared for individual patients pursuant to a valid prescription from a licensed prescriber.
Using the forms
You agree to submit accurate information and to submit information about another person only with their permission. Do not use the forms for emergencies. Automated submissions, scraping and attempts to interfere with the site are not permitted.

The text, design and logo on this site belong to Pure Health Compounding, LLC. You may read and print pages for personal, non-commercial use.
